The staff of Lifestyle Support Services, Inc. has 30+ years of combined experience in both Mental Health & Intellectual Developmental Disabilities. There is a realization by the PA Department of Human Services that individuals with mental retardation are frequently challenged by mental illness. Given this, there is an increasing need to provide a truly integrated community model offering intensive clinical supports affording persons continued opportunities to remain in their communities in the least restrictive environments.

 

Lifestyle Support Services, Inc. currently provides services to the following counties: Blair/Cambria, Bedford/Somerset, Clearfield/Jefferson, Indiana/Armstrong, & Forest/Warren.

Residential

Home-based services are services offered to an individual residing at home or in an alternative setting. The purpose of home-based services is to offer supports to an individual and, in many instances, to his or her family. These supports enable & empower the individual to develop or maintain skills necessary in everyday living.

The individual and his or her support team, including the support coordinator, home-based coordinator, and family or provider, determine specific services to be provided. Outcomes or goals are person-centered, meaning plans are developed based on the needs of each individual. Some outcomes, for example, may focus on increasing participation in community events or to develop social skills. Some outcomes are community-based while others are specific to in-home needs.

Lifestyle Support Services, Inc. provides flexible home-based services 7 days a week and delivers these services with care, compassion, & respect. The objectives of the home-based program are to:

 

➔ Assist each person in establishing a safe, healthy, & comfortable lifestyle suitable to his/her needs

➔ Encourage each person to be a participating and contributing member of the community

➔ Assist the individual in building relationships with family, friends, & others

➔ Provide opportunities for greater independence and the skills that will enable each person to self-advocate

➔ To provide quality services meeting and exceeding recognized and respected standards for service provision

Lifestyle Support Services, Inc. currently provides services to numerous residential sites throughout Blair, Cambria, Indiana, Clearfield-Jefferson, & Warren counties.

The community living arrangements offered by Lifestyle Support Services, Inc. provide housing, services, and support specific to the needs of each individual. The focus of this versatile program is to enable the individual to live his or her life to their fullest with assistance and comprehensive services as necessary. Each individual has ongoing opportunities in his or her home and community to participate in activities that provide social interaction as well as the development of friendships with others.

Behavioral

Behavioral

Lifestyle Support Services has Master's level Clinicians & Behavior Specialists that work one on one with the individuals to teach them skills in becoming more successful in their daily lives.

Services that are provided include:

➔ A Functional Behavior Assessment of presenting issues

➔ Provisional training to individuals, staff, relatives, and/or caregivers.

➔ Observations & collaboration with the individual, their family, & treatment team for the purpose of developing a Behavioral Support Plan that includes positive practices.

➔ Completion of required paperwork related to data collection, progress reporting, & development of annual planning material.

Behavior Specialist

 

Lifestyle Support Services has an in-house Master’s level Behavior Specialist that collects and analyzes behavioral data and based on the data, develops, implements, & trains staff on strategies to best address the behavioral needs by ongoing evaluations. The Behavior Specialist works with the residential service workers in educating them about the individual whom they work to support and teaching de-escalation techniques that are the most effective for the individual. The Behavior Specialist also completes training with the residential service workers on the individual Safety/Behavior Support Plans and on strategies to best address and meet the behavioral needs by ongoing evaluation of the individuals that are served agency-wide. The Behavior Specialist collects & analyzes data, completes quarterly reports, develops fade plans that support optimal functioning, & creates treatment plans to support the individuals in developing coping skills to assist them in being more successful. The Master’s level Behavior Specialist is certified in completing Functional Behavior Assessments and is a trainer in the use of Intensive Systems Therapy. The Behavior Specialist and members of the treatment team also meet weekly to debrief restraints to analyze the situation and determine if additional replacement strategies or training are necessary for the individual or team.
